State Library of NSW SL magazine Survey 2016 Promotion 
TERMS AND CONDITIONS

  1. Terms - These terms and conditions govern entry to, and participation in, the promotion. To the extent necessary or appropriate, information about this promotion and eligibility to participate in the promotion form part of the terms and conditions governing the promotion.
     
  2. Promoter - The promoter is State Library of NSW (ABN 19 665 924 697) of Macquarie Street, Sydney NSW 2000 (State Library of NSW).
     
  3. Promotion Period - The promotion commences on Monday 22 August 2016 and ends on Wednesday 30 November 2016 (Promotion Period).
     
  4. Eligibility - The promotion is only open to people (Eligible Participant) who complete the State Library of NSW’s SL magazine Survey 2016.
     
  5. State Library of NSW staff ineligible – State Library of NSW staff are ineligible and may not participate in this promotion.
     
  6. Promotion - The promotion involves a prize draw open to people who participate in the State Library of NSW SL magazine Survey 2016 Promotion. Participants must complete the survey in full and indicate in the survey they would like to enter the promotion by ticking the appropriate box. The prize draw is only open to an Eligible Participant. There is one main prize of $200 in Library Shop vouchers. The prize is not transferable or exchangeable and is not redeemable for cash. State Library of NSW reserves the right to substitute the prize(s) with an alternative item that State Library of NSW considers to be of equivalent value.
     
  7. Entry - Entry into the promotion is taken to be acceptance of these terms and conditions. State Library of NSW may at its discretion refuse to award the prize to an Eligible Participant who fails to comply with these terms and conditions.
     
  8. Only one entry per person throughout the Promotion Period will be permitted.
     
  9. Winner notification – The prize will be drawn on Wednesday 21 December 2016. The winner will be randomly selected. State Library of NSW will notify the prize winner by email or phone.
     
  10. Decision final - State Library of NSW's decision is final and no correspondence will be entered into.
     
  11. Delivered surveys - All surveys delivered to State Library of NSW as part of the State Library of NSW SL magazine Survey 2016 become State Library of NSW’s property.
     
  12. No liability - State Library of NSW, its contractors, employees and agents, will not be liable for any claims, losses, damages, injuries, costs and expenses suffered sustained or incurred (including but not limited to indirect or consequential loss) as a result of, arising out of or in any way connected with the promotion, except for liability that cannot be excluded by law.
     
  13. Eligible Participant disqualification - If State Library of NSW considers that this promotion cannot be conducted as planned for any reason, including unauthorised intervention, fraud, or any other causes beyond State Library of NSW’s control, which corrupts or affects the administration security, fairness, integrity or proper conduct of this promotion, State Library of NSW reserves the right (subject to any applicable law) in its sole discretion to disqualify any Eligible Participant who tampers with the entry process.
     
  14. Recommencement - If there is an event beyond State Library of NSW's control which prevents or hinders the conduct of the promotion or State Library of NSW's ability to deliver the promotion, then State Library of NSW may in its discretion cancel the promotion or recommence it under the same conditions.
     
  15. Privacy Notice - State Library of NSW is collecting your personal information so you can enter, and it can administer, this promotion. Without your personal information State Library of NSW cannot enter you in the promotion. Subject to some exceptions allowed by law, you may request access to your personal information while State Library of NSW stores it. State Library of NSW will assess your request in accordance with the law and tell you why if access is denied. A request to access, update or correct any information should be directed to the research officer via email research.library@sl.nsw.gov.au. The winner’s name will not be used outside State Library of NSW publications and website without notice unless otherwise advised by the entrant.
